How can patients get assistance in obtaining Mirena®?

Patients or providers should call 1-877-393-9071 to talk to a patient case coordinator. The coordinator can answer any questions regarding how to apply for assistance and can also assess whether it appears appropriate to apply for assistance. If appropriate, an application will be sent to the patient or healthcare provider.

To be considered for assistance, the patient and her healthcare provider must each complete his/her part of the one-page application and submit it by fax or mail.

A patient case coordinator will review the application and determine eligibility within two business days of receiving a complete application, which includes patient income information.

Both the patient and her healthcare provider will be notified in writing regarding whether the patient qualifies for assistance.

If the patient qualifies for assistance, Mirena will be shipped to the patient's healthcare provider or clinic.
